Registration Information

Steps to Register

Please note: this process is required for every athletic season and must be done on a computer

  1. Log into MySchoolBucks
  2. Click on School Store
  3. Click on Browse All Items
  4. Filter by Cresthill Middle School and Athletics
  5. Scan activities until you see you the correct icon for the sport you're playing (Example - "CMS - 7th/8th Grade Cross Country 20-21")
  6. Follow all of the prompts from there including links Physical Form, Fees, etc.

Physicals

Please note: All DCSD student-athletes are required to have an up-to-date physical on file in order to participate.

Sports Physical Form

If you need a hard copy of the physical and are unable to print it off, please stop by the Cresthill Main Office to get one.

Please drop off your completed form to our Athletics Secretary, Stephanie Young, or email the completed forms to Stephanie Young at stephanie.young@dcsdk12.org.
